'Systemic failures' cost Schenectady £7,000

Chemicals manufacturer Schenectady Europe has been fined £5,000 with £1,959 costs after an acid gas cloud escaped from its Four Ashes works near Wolverhampton. The incident was the latest in a series of incidents at the site caused by "systemic failures in training and procedures."
