Oil companies panned over refinery explosion

The explosion and fires at the Texaco/Gulf oil refinery in Milford Haven in 1994 were caused by "a combination of failures in management, equipment and control systems," according to a report by the Health and Safety Executive (HSE).1 Shortcomings in inspection and maintenance of instruments and pipes and the firms' haste to restart a plant after a lightning strike also contributed to the disaster.
