BP gets a roasting over safety failures at Grangemouth

Fragmented management and a failure to address deteriorating performance were responsible for a catalogue of errors which led to a power failure, explosion and fire at BP's Grangemouth petrochemical complex in 2000, according to a report by the Health and Safety Executive.1 The company's plans for preventing further major accidents at the site are being withheld from the public on security grounds.
