"Elementary mistakes" led to Hickson disaster

"Fundamental errors and incorrect assumptions" led to the deaths of five people in an explosion at Hickson & Welch's chemical manufacturing site in Castleford, Yorkshire, according to a report by the Health and Safety Executive (HSE).1 The firm failed to carry out a thorough hazard assessment and to ensure basic procedures such as safe systems of work for operation and maintenance of plant.
