Major hazard reports under scrutiny after refinery blast
Lightning strikes appear to have contributed to a serious explosion at Texaco's Pembroke refinery which caused damage to buildings two miles away and has raised questions over the plant's safety record. Meanwhile, Friends of the Earth (FoE) has drawn attention to the secrecy surrounding safety reports submitted to the Health and Safety Executive (HSE) under legislation on major accident hazards - and also to the snail's pace at which they have been assessed by the HSE.
