HSE pinpoints Octel's failings over chemical blaze

Risk assessment and maintenance procedures at Associated Octel's chemical works in Cheshire have been strongly criticised by a Health and Safety Executive (HSE) report on a major chemical leak and fire at the site in February 1994.1 But the incident has also raised questions once again about the adequacy of the HSE's scrutiny of companies' safety reports on "major hazard" installations.
